아동

● Baby feeding

배고픔

- smacking(쩝쩝), 손빨기, 손 얼굴쪽으로 가면 돌림, cooling sound, 가슴찾기위해 고개돌림, 혀내밀기, crying(마지막행동)

배부름

-입닫기, 고개돌림(가슴반대), 먹다 딴짓(아빠쳐다보기)

 

● rectal enema, irrigation

온도 : 실온, 체온 but 뜨거운 것 X

enema : Infant 150-200cc, 1inch(2.5cm)

irrigation : 10~20cc/kg(깨끗해질때까지), 4inch이상, 60cc syringe에 20cc 담아 반복

 

● Celiac disease

- 소장의 gluten이 반복 무리가서 소장이 변함

- Family Hx, 자가면역 → gluten 제한식이 필요

Dx)

- Blood : Ig G, A ↑ (screening, 치료상태확인)

- jejunal biopsy (confirm)

wheat rey(호밀) oat(귀리) barley(보리) cereal fillers	rice corn millet(수수) potato buckwheat(메밀) tapioca

- 우유 복용가능 but diarrhea 박복시 lactose intolerance 발생 가능

 

● Lactose Intolerance

- type1, congenital : 조산(lactase X)

- type2, adult : 5y↑

- type3, secondary : 심한설사 후(lactase 빠짐)

milk allergy : milk protein, 2y↓ / LI : carbohydrate, 나이↑

● constipation 연령(소아)

- infant : 분유→solid

- Toddler : sugar & milk 多 → 대장, 수분재흡수↑

- schoolage : 학교생활로인한 배변제한 → 배변 습관

 

● Intussusception(장중첩증)

Sx)

- currant jelly stools

- 중첩 위 : hyperactive, 중첩 아래 : hypoacitve bowel sound

- vomiting

Tx)

- hydrostatic : reduction with barium(정상변 시 중지, 자동으로 풀림), perforatoin 예방

 

● megacolon

Sx)

- constipation, diarrhea, abd.distention, failure to thrive(성장지연)

 

● Hirschsprung disease

- nerve missing정도에 따라

mild : 관리 후 적정 op 나이 때 수술 / severe : op 후 증상 완화 후 재결합

● lead poisoning

risk)

- 오래된 집

- paint(oil 안좋음/water(latex, 괜찮)

- 유리색 직업, 장난감, 직장 등등

G-I, inhalation intake / kidney 배출

 

● PO 多 복용

- Poison control center : PCC이용

 

● hernia OP

- diaper교환(infection 예방), pain : discomfort수준

 

● Rabies Human

- Ig G : 1/2 주사, 1/2 상처

- vaccine : 6번(ASAP, 3d, 7d, 14d, 28d, 90d)

 

● 시력검사

- allen picture cards(그림)

- tubling E

- snellen letter charts(글자)

- 7y 거의 발달

● ostitis Media(OM, 중이염)

- ototoxide 없는 anti 사용 (ototoxide 있는 anti : gentamycin)

- hearing 문제 반복 감염

 

● hearing test

- 발달단계에 따른 확인(옹알이, 소리반응, tunning fork(5y↑)

- primary caregiver 중요

- conductive : OM

- sensori neural : 메니에르, loud music

● tonsillectomy

- WBC↑ infection 시 OP hold

- prone position(의식 X) / fowler, sitting position & side-lying (의식 O) / X supine

- frequent swallowing(bleeding sign)

- PRN) ice collar

- liquid diet

- cool water(ice tea, red계통 X bleeding과 혼동)

- dilluated apple juice 가능

 

● Epistaxis(nose bleeding)
