Dz-GI

● small intestine obstrution Sx

- severe abd.pain, cramping

- projectal vomiting

- abd distention(심하진 않음) but 대장일 경우 심함, 막혀서 vomiting, diarrhea X

 

● GERD

- diet : fiber↑, fat↓ 소화속도↑

- X eat or drinking 2hrs before bedtime

- X tight cloth

- HOB 6~8 inch. 30도 block고정

- sit after 30min after meal

- small, frequently

 

● Ileostomy

- stool : liquid, semiliquid

- stoma : pink, bright red & shimy

- gas odor (yogurt odor↓)

- applience : 1/2~1/3 full empty, q5~7day bag change, 물과 비누로 clean, 1/6~1/8 inch 교환시 테두리

 

Cx)

- electrolyte, fluid imbalance, obstruction : low residue diet ( X : raw fruit, veges)

- fluid↑, vit, mineral ↑

- low residual

- odr gas↓(yogurt/↑:milk, egg, fish, nut etc.)

● Dumping syndrome

- 음식을 천천히 소화할 수 있게

- protein, fat > CHO(complex > simple)

- fluid not with meal, between meal

- X sugar, salt(너무 달고,짜면 삼투압↑, 소화↑)

- frequent, small

- rest 30min, lie down 30min after meal

 

● small bowel obstruction

- severe cramping abd. pain

- projective vomiting

- distension

- high pitch bowel sound, frequently

● pancreatitis

- abd.pain, midepigastric or LUQ

- radiatied back

- abd. tenderness

- N/V not relieved by vomiting

- Wt loss

- lipase, amylase 혈중 ↑

- cullen(배꼽), Turner(옆구리) sign + 멍

- Ca↓ (tetany 가장 위험한 증상)
