Distal radius ORIF 손목뼈 골절 수술

: Bovie, Suction line(PVC), Spoid, 50cc syringe, K-wire, 막holder, wire cutter, 무영등 손잡이, Bowl(two), 4’ STN, 4’EB(2), 4’CB, 15번 Mess(3), Vicryl 2/0(1), Vicryl 3/0(2), Skin stapler(R), Pen, C-arm포

PRN
: Bone clip(소), Homann(mini)

Procedure
<Incision>
-Pen으로 표시함
-15번 Mess로 Incision함
-Metzenbaum(중) + Adson Tissue forcep으로 Subcutaenous dissection함
-15번 Mess로 fascia Incision함
- Metzenbaum(중) + Adson Tissue forcpe으로 muscle dissection함
- freer or Periosteal로 Bone막? 걷어내면서 Bone 노출 시킴

<Reduction>
- 50cc syringe로 Fx site 보이게 irrigation해서 씻어냄
-Freer 또는 손을 당기거나 꺽어서 Bone Reduction 시킴
(Fx에 따라 Reduction 방법들이 있는데 그 부분까지는 아직 정확히 잘 모르겠음)
- Novosis(DBM) 소Bowl에 넣은채로 부수지 말고 전달함(그대로 떠서 Bone Fx 사이에 밀어 넣음)

<Plate 고정>
- Plate Size(Standard / Large) + 길이(2,3,4 Hole) Bone 사이즈 보면서 알맞은 크기 Plate 찾음
- Locking sleeve 끼워서(Hole 가장 끝, Proximal쪽) 전달함
(주의, Cortical 부분에도 어찌 들어가지니 주의해서 Locking 홈 있는 구멍에 끼워서 전달하기)
- K-wire 1.2 size 절반으로 잘라서 Plate 임시 고정용으로 사용함
(경우에 따라 4개 고정하기도 함)
- Proximal(Bit @2.0) Cortal hole(2H 위치) 먼저 고정함

- Proximal locking cannulated screw 1개 먼저 고정 후 Distal로 올라감
- Distal(Bit @1.8) locking cannulated screw 고정함
- Distal sleeve 말고 Variable sleeve(Bit @1.8) 이용해서 distal 끝부분 2개 정도 locking cannulated screw 박음
- 나머지 screw 전부 다 고정함

<Irrigation, Suture 및 마무리 Dressing>
- 타우로린+크린조 Spoid로 irrigation함
- ?? Hemovac SS100M Needle 90도 정도 구부려서 전달함
(Y-closure로 고정함, Hemovac 바로 OPEN)
- Vicryl 2/0(lapa용) muscle & fascia? suture함

- Vicryl 3/0 subcutaneous suture함
- Stapler Regular skin 닫음
- 프로테스칼(유착방지제) 넣음
- Betadine ball dressing 후 4x4 Gauze dressing함
- 4’CB + 4’EB + Bongips 고정함
