Tension band wiring patella Fx ORIF

주먹 감자 같이 생긴 Patella가 부러질 경우
주로, Tension band wiring을 이용해서 고정한다
Tension band wiring은 말 그대로
구부러지는 철사를 감아서
쪼여주면서 Tension(긴장)을 준 상태로
부러진 Patella Bone 조각들을 모아주는 것이다

Drape
: Alcohol -> Betadine 소독함
: Betadine ball로 추가 소독함(Ring forcep, Long kelly)
: Medium drape 2개, 1개씩 Tourniquet 아래쪽 1개 깔고 위쪽에 Tourniquet 덮으면서 붙임
: Utililty drape 1개 돌려서 아래쪽 남은 Tourniquet 붙임
: U-drape 아래쪽으로 붙임
: 대공으로 발 넣고 펼침

Procedure)

<Incision>
-Pen으로 표시 후 15번 mess incision
-Metzenbaum + Tissue forcep
-15번 mess로 patella bone 나올 때까지 들어감
(Army, Self로 벌림)

<Patella Bone Reduction>
-Irrigation함
+NOVOSIS(뼈 대체제?) 사용할 경우 부러진 Bone 사이에 넣어줌(소 Bowl)
-Bone clip(소/중)으로 Patella bone 잡아서 Reduction함

<Tension band wiring>
-K-wire 1.6 Patella bone distal 쪽에서 Proximal 방향으로 평행하게 2개 박음
-Roll wire 18G(1번) 60cm정도로 준비함
-18G spinal needle로 K-wire 아래쪽?(Distal)으로 통과시켜서 Roll wire 통과시켜서 빼냄

-18G spinal needle로 K-wire 윗쪽?(Proximal) 통과시킴
-K-wire에 걸쳐서 Roll wire 'X'자 모양이 되게 만들음
-Wire holder로 Roll wire 꽈배기처럼 돌려서 tension 주기 시작함
-tension 정도 Perio or Freer로 확인하면서 wire 돌려서 tension 적절할 때까지 돌림(너무 무리할 경우 wire 끊어지기도 하니 적당히,,,)

-충분히 tension 들어가면 Wire cutter로 말아올린 roll wire 잘라냄
-Suction tip으로 K-wire 아래/윗쪽 꺽음
-아래쪽 먼저 Wire cutter로 잘라냄
-위쪽 방향 보면서 suction tip으로 k-wire 구부리고 C-arm으로 길이 보면서 wire 자름
-Wire holder로 아래쪽 잡고 mosquito로 위쪽 잡아서 자르고 남아서 튀어나온 K-wire 부분 돌려서 파묻음(필요시 Mess로 파묻을 곳 incision함)
-Impactor mallet으로 roll wire 꽈배기 남아서 튀어나온 부분 쳐서 넣어줌

-Irrigation함
-Vicryl 1번 + Tissue forcep Muscle?, Fascia? suture함
-Vicryl 2/0 + Adson Tissue forcep subcutaneous suture함
-Skin stapler W - Protescal - 4x4 Wet gauze - Dry gauze
-4x4 Gauze - 6'CB - 6'EB - Bongips(cylinder) - RASSET(ice pack 보조기)
