환자 자세 Position

: lateral position 

수술 전 소독

: Two Bowel 2개 이용함
Betadine, Alcohol, Betadine soap 3가지로 소독함

수술 전 Drape

: Ring forcep으로 Towel 찝어서 닦음
Utility 4개 붙임 - U-drape 깔음 - 대공 깔음 - 중포?로 위/아래 깔음?​

Scrub 수술 전 준비

Saw 드릴
: 드릴에 Saw 물려놓음
모양 맞춘 다음 시계방향으로 돌려서 끼움

: Tab, Needle, Blade Count함
Boxer/Start Reamer/Cork screw/Sizer
Reamer/Reamer handle/Trial head(S/M/L)

: BPHA 기구 준비함
Sizer, Boxer, Cork screw, First reamer, Reamer, Reamer handle, Trial head(Short? Small 회색/ Medium 초록 / Long , 파랑), Trial Cup(Femur head Size별로), Head Impactor, Real stem impactor, 찬니? Retractor

BPHA 수술 과정

1.Pen으로 incision 부위 그림
2.10번 Blade 4번 mess handle로 skin incision함
3. Bovie, Dressing tissue forcep 이용해서 subcutaneous cutting함
점점 들어가면서 Army, Richardson(대) 사용함
4. Metzenbaum scissor(대)로 dissection 함
5. 찬니 retarctor로 걸고 Vicryl 1번 tagging sutrue 후 mosquito로 물어놓ㅇ므
6. Capsule incision 후 mess handle 4번-> 7번(길이 김), Bovie 중팁(상황보고 바꿔달라 함) 변경해서 줌
7. Homann(중), Cobb's로 Femoral head 나올 때까지 Bovie로 cutting 및 Rongeur로 정리함
8. Saw로 Femoral neck 자름, 자를 때 Protector 위로 올리기
+ 필요시 osteotome (중, str) + mallet 주기

9. Cork screw로 Femoral head 끼워서 뺴냄, Rongeur로 Bone 뜯어서 정리함(받아서 모아두기)
+ size 확인함
10. Boxer로 stem 들어갈 자리 치고 First reamer로 갈아냄
11. 1번 Reamer부터 단단히 고정되는 사이즈 될 때까지 Reaming 함
12. Trial Head, Cup Reamer stem에 끼워서 환자 다리 길이 확인함
13. 안 맞으면 사이즈 바꿔가면서 맞춰나감
14. 원하는 사이즈 되면 Real stem open함(위에 플라스틱 및 부속품만 뺴고 통 자체로 Dr에게 전달하기)
- Real Stem 과 Stem impactor 전달하기
15. Real stem 들어가고 Trial로 사이즈 다시 확인해봄

16. Size 맞으면 Real open 해서 만지지 말고 과장님 전달하기
17. Real 끼우고 impactor로 밀어 넣음, 환자 다리 길이 확인함
18. Ethibond 2개 정도 Capsule suture 후 mosquito 물어놓음 
19. K-wire 2.0 drill로 Bone 구멍 내서 canal 만든 다음 꼰 wire(끝에 구멍에 들어가게 찌부시켜 놓기) 전달함 - mosquito or Adson Tissue forcep으로 통과한 꼰 Wire 구멍 넓히고 사이에 Ethibond 넣고 suture 마무리함
+ Barovac 필요시 capsule suture 후 삽입함
++ 1차 카운트함!!(Tab 바닥에 다 내리기)
20. Vicryl 1번 Suture - (2차 카운트) Vicryl 2/0 sutrue - subcutaenous stapler - (3차 카운트) skin stapler(R) suture 함
21. 4x8 gauze + fixroll 고정 후 CB(1)+ A-EB(2) dressing 함
