담석증 개복술 수술과정 OPEN cholecystectomy

Glove, Gauze, #3, #5 stick?, long?, penrose drain(2), 20번 mess, Dexon 2/0, Nylon 4/0(2), 50cc syringe(3), 18G needle, die catheter, right angle sharp, Pott's forcep(2)
Prep
: Long right angle, Long needle holder

Procedure)
1.20번 mess로 Incision 낸 후 Bovie, Kelly, Army를 이용해서 Muscle층까지 incision냄
2. Fascia - Peritonium(tissue + tissue), mess - (kelly + kelly), Metzenbaum scissor
3. kim's & G-S self, Long pin(2), Richardson, Deaver 이용

4. GB를 liver쪽과 박리할 경우
-mess incision 후 suction tip(cap X) 흡인, 3번 Long silk suture(내장용), mosquito clamp
-50cc 주사기 + 18G needle로 흡인
-GB babcock으로 clamp한 다음 Right angle, kelly(중/tonsil), Bovie, Metzenbaum sicssor로 박리함, 이때 3번 Long silk tie로 사용
5. 박리시 duct 나오면 Long 5번 silk tie or suture
6. 박리 후 die(20cc) + N/S(30cc) mix한 50cc syringe die catheter 삽입 후 die 삽입
7. drain insert시 mess, bovie, kelly (skin suture 준비)

8. Dexon 1/0 peritonium suture, irrigation
9. Fascia Maxon 2/0 suture
10. Muscle Dexon 2/0 suture
11. Skin Nylon 4/0 suture
+ Drain insert시 drain kelly에 clamp.
이때 구멍나지 않은 곳 clamp(안에서 밖으로 insert 함)
